ATTENDANCE POLICY: Attendance at all auditions, callbacks, tech and dress rehearsals, and show times are mandatory. Participation in this production should be the participant's highest priority over other activities. In the event of unavoidable schedule conflicts, please notify the directors at least 24 hours in advance.

By completing I hereby certify that I am a parent or legal guardian of the child participant (or I am at least 18 years of age if I am the participant) and that all of the information provided is correct and complete. I recognize that stageGR will rely on that information. I will be responsible for the cost of any emergency medical care provided to the participant should stageGR instructors or staff, in their judgement, call 911 on behalf of the participant in the event of an emergency. I understand that by this authorization to participate in a stageGR program, I consent to allow the participant to be photographed and/or video/audio recorded. These photographs and video/audio recordings may be used by stageGR in future promotional efforts without requiring further specific written permission from me.
